The outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth.
A survey by the WHO and the health ministry on students in 11 localities revealed that the rate of using e-cigarettes among students has been consistently rising among certain age groups. They are addictive as they contain nicotine, can cause diseases like cancer, and affect one’s mental health. With the menthol cigarette ban uncertain, the path for low-risk tobacco product authorization needs to be reimagined to nationally increase MRTP availability, access, and appeal among people who use menthol cigarettes. For example, one study among people who smoke showed that hypothetical general claims about completely switching to a heated tobacco MRTP outperformed specific claims—such as switching from cigarettes will reduce the risk of lung cancer and exposure to arsenic.
For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. This was significant because Chantix, at one point, had a “black box warning” after reports linking the drug to psychiatric side effects. That warning was dropped in 2016 after a very large study showed the drug to be safe, but Toll and Fucito said the stigma of the warning remains in the minds of both health care providers and the general public. The age-standardized prevalence of current e-cigarette use among individuals who reported never using combustible cigarettes was 2.9% (95% CI, 2.8%-3.1%). The prevalence was higher among individuals who reported former combustible cigarette use, at 17.2% (95% CI, 16.5%-18.0%), and current combustible cigarette use, at 17.9% (95% CI, 17.1%-18.7%). The age-standardized prevalence of daily e-cigarette use by smoking status showed similar patterns (Table 2). In a recent study, about 18% of people who switched to vaping had been able to quit smoking. That’s about twice as many people as those who used other methods to quit smoking. Of people who used other methods, 91% kicked nicotine products altogether.
Vaping exposes you to some of the same chemicals that cigarette smoking does. Vaping while pregnant can cause low birth weight, lung damage and brain damage in the developing fetus. Accidental exposure of children to ENDS e-liquids pose serious risks as devices may leak or children may swallow the poisonous e-liquid. In many e-cigarettes, puffing activates the battery-powered heating device, which vaporizes the liquid in the cartridge.
Still, it’s far from safe and can lead to nicotine addiction and potentially fatal lung disease. Electronic cigarettes—or e-cigarettes—are battery-powered devices that convert a liquid, usually with nicotine, into an inhalable aerosol. First introduced in the United States about 15 years ago, e-cigarettes have evolved from disposable, single-use products to more sophisticated devices that can be refilled and customized (fig. 1). One of the largest studies done on this was published in The New England Journal of Medicine in 2019. The study, which included 886 randomized participants, compared e-cigarettes and nicotine replacement therapy, which included patches, gum, lozenges, nasal spray, and mouth spray.
School initiated a vape detection pilot program, emphasizing the administration’s commitment to prioritizing student health and safety. The prognosis depends on how much nicotine was taken and how quickly treatment was started. If a person is able to survive during the first four hours after poisoning, they’re usually likely to recover.
ENDS emissions therefore pose potential risks to both users and non-users. Some vape shops may have legal responsibilities as both manufacturers and retailers of tobacco products. These products use an “e-liquid” that usually contains nicotine derived from tobacco, as well as flavorings, propylene glycol, vegetable glycerin, and other ingredients.
Cig-a-likes may also contain low levels of cadmium, a toxic metal also found in cigarette smoke that can cause breathing problems and disease.11 More research is needed on the health consequences of repeated exposure to these chemicals. There are also reports of lung illnesses and deaths related to inhalation of certain vaping oils into the lungs, which have no way to filter out toxic ingredients. As the menthol ban hangs in limbo, people who cannot quit smoking those products altogether could be advised to shift to less harmful alternatives. These so-called modified risk tobacco products (MRTPs) are less dangerous than traditional cigarettes, according to the Food and Drug Administration (FDA). But they have generated limited sales, in part because they use ineffective advertising and must undergo an onerous authorization process.
Even if you have used tobacco yourself, your children will listen if you discuss your struggles with nicotine addiction. Be clear that you don’t approve of them smoking or using e-cigarettes, and that you expect them to live tobacco-free. Today, more high school students use e-cigarettes than regular cigarettes. The use of e-cigarettes is higher among high school students than adults.
E-cigarette aerosol has been found to contain lung irritants, cancer-causing chemicals, heavy metals, volatile organic compounds, and usually nicotine. The deeming regulation includes requirements for pre-market review for e-cigarettes as new tobacco products. While some e-cigarettes may help some people quit, the diversity of products as well as the variations in quality and the lack of regulation make determining the potential of any particular product as a quit aid difficult. Some smokers have switched to e-cigarettes or used them to quit tobacco completely, however, e-cigarettes, unlike FDA-approved cessation therapies, lack an evidence base that demonstrates their safety and efficacy. Truth Initiative supports regulation that encourages the development of consistently less harmful nicotine delivery alternatives that allow smokers to quit tobacco altogether or switch completely to a much less harmful, well-regulated product. In addition to e-cigarette products, tobacco companies have introduced new products that work by heating tobacco instead of burning it.
Use of electronic cigarettes (e-cigarettes) impairs indoor air quality and increases FeNO levels of e-cigarette consumers. E-cigarette manufacturers are using marketing tactics used by the tobacco industry to lure kids to conventional cigarettes.
