Nicotine e-cigarettes: considerations for healthcare providers

Electronic cigarettes (e-cigarettes) and other vapor products are used to heat and aerosolize (turn into particles) liquid known as e-juice (vape juice) containing highly addictive nicotine or other substances that a person can inhale into their lungs. E-cigarettes emit toxins and harmful ultrafine particles, both of which pose potential health risks similar to secondhand smoke. Many nicotine refill bottles or cartridges are not adequately packaged to prevent children from coming into contact with or accidentally ingesting toxic amounts of the vaping liquids. Studies also have shown that vaping may cause respiratory and cardiac changes (Gotts et al., 2019; Qasim et al., 2017). We work with community partners to raise awareness of the harms of e-cigarettes and vaping and flavored tobacco products that attract youth, through the Unhyped and CounterBalance campaigns.
Use of electronic cigarettes (e-cigarettes) impairs indoor air quality and increases FeNO levels of e-cigarette consumers. E-cigarette manufacturers are using marketing tactics used by the tobacco industry to lure kids to conventional cigarettes. Their tactics include slick magazine ads, sponsorship of concerts and auto races, celebrity endorsements and sweet, colorful flavors.21 In addition, e-cigarettes are often aggressively placed in convenience stores near candy.
Unlike traditional cigarettes, e-cigarettes are made up of a battery-operated heating part — a cartridge (unit) that typically holds nicotine and other chemicals that change into a chemical-filled aerosol when heated. As a result of the FDA’s missed deadlines and inadequate enforcement, flavored e-cigarettes remain widely available online and in stores across the country. Every day flavored e-cigarettes remain on the market, our kids remain at risk. The FDA must act without further delay to remove all flavored e-cigarettes from the market. Despite not authorizing a single synthetic nicotine product, it has allowed these products – including e-cigarettes in kid-friendly flavors – to stay on the market well past the July 13 deadline.
Nicotine exposure during this vital time can affect brain development in subtle and important ways. Their brains are still developing and forming the structure and connections necessary for the mature behavior of adulthood. Research published in 2017 found that the high temperatures needed to form the mist for vaping can create dozens of toxic chemicals, such as formaldehyde, which is thought to cause cancer. There are currently 78 licensed tobacco retailers in unincorporated Sonoma County, representing 23 percent of the total tobacco retailers throughout the county including cities. The heating element aerosolizes the liquid for the inhalation of the liquid nicotine or other contents. E-cigarettes are known by many different names, including “vapes,” “e-cigs,” “puff bars,” and “electronic nicotine delivery systems (ENDS).” Since e-cigarettes are a relatively new product, there is still a lot of research needed to be done on the long-term health effects. While studies have shown that e-cigarette aerosol is less harmful than traditional tobacco smoke, that does not mean that it is safe.
Although evidence is lacking, e-cigarettes for inhalation of nicotine may be beneficial in reducing adverse health effects related to the use of combustible tobacco products. However, e-cigarettes are not FDA approved as a smoking cessation tool and the amount of nicotine and other substances a person inhales from each nicotine cartridge remains unclear. When FDA “deemed” e-cigarettes as part of its jurisdiction in 2016, it gave e-cigarettes that were currently on the market two years to prepare premarket applications (known as PMTAs). These applications are what FDA uses to to determine whether new tobacco products are “appropriate for the protection of public health” before they are allowed on the market.

In January 2020, the FDA prioritized enforcement against the sale of most candy or fruit flavors in “closed pod” refillable e-cigarettes, like the popular brand JUUL, but exempted disposable flavored e-cigarettes including newer products like Puff Bar. The e-cigarette industry has responded by offering newer products that aren’t covered by existing enforcement guidelines. In July 2020, the FDA issued warnings to e-cigarette retailers and manufacturers, including Puff Bar, to remove their flavored disposable e-cigarettes and youth appealing e-liquid products from the market. In young people, the amount of nicotine needed to establish an addiction has been estimated at around 5 mg a day, or roughly one-quarter of an e-cigarette pod.

And more than a quarter (27.6%) of current youth e-cigarette users say they use an e-cigarette product every day. These electronic devices, also known as e-cigarettes, vapes, vape pens, personal vaporizers, e-cigars, pod systems, mods and e-hookah, are not a safe alternative to cigarette smoking. The report, published in the Morbidity and Mortality Weekly Report, was based on data from the 2021 National Youth Tobacco Survey (NYTS), a cross-sectional, self-administered survey of U.S. middle (grades 6–8) and high (grades 9–12) school students. The study assessed current (used on one or more of the past 30 days) e-cigarette use; frequency of use; and use by device type, flavors and usual brand. Centers for Disease Control and Prevention (CDC) estimated that more than 2 million U.S. middle and high school students reported currently using e-cigarettes in 2021, with more than 8 in 10 of those youth using flavored e-cigarettes. Research from The Johns Hopkins University on vape ingredients published in October 2021 reveals thousands of chemical ingredients in vape products, most of which are not yet identified.
The ingredients, including nicotine levels, vary between brands and flavors, but many products are not thoroughly or accurately labeled. From introducing appealing flavors to offering college scholarships, manufacturers and sellers of e-cigarettes aggressively target young people. There are few federal restrictions on e-cigarette marketing, allowing companies to promote their products through traditional outlets — such as TV and radio — despite a ban in 1971 on cigarette advertising in both outlets to reduce cigarette marketing to children. E-cigarette companies also take advantage of other marketing outlets, including the internet, retail environments, and recreational venues and events. Nicotine is an addictive substance, but its level of addictiveness can vary substantially depending on its mode of delivery. Nicotine delivered by the combustion of tobacco is the most addictive form.12 The rise in popularity of e-cigarettes that can deliver levels of nicotine similar to combustible cigarettes is causing concern about the potential risk for addiction.

At minimum, this outbreak dramatically demonstrates the dangers of an unregulated market in inhaled substances with no premarket review for consumer safety. E-cigarettes are hooking a new generation on nicotine – putting millions of kids at risk and threatening decades of progress in reducing youth tobacco use. It’s a nationwide crisis of youth addiction, fueled by thousands of kid-friendly flavors and massive doses of nicotine.
Not only is there evidence of mislabelling of nicotine content among refills labelled as nicotine-free, but there also seems to be a history of poor labelling accuracy in nicotine-containing e-liquids [37, 38]. Cancer is definitely a concern, given that vaping introduces a host of chemicals into the lungs. But vaping products haven’t been around long enough for us to learn whether or not they cause cancer. And, while it’s safe when taken orally as a supplement or used on the skin, it’s likely an irritant when inhaled. It’s been found in the lungs of people with severe, vaping-related damage. Research on health effects of secondhand exposure to e-cigarettes is still emerging.
The increase in e-cigarette use (also called vaping) by kids and young people in recent years is a serious public health threat. Since being introduced to the U.S. market in 2007, e-cigarette use among youth has increased to epidemic levels (Surgeon General’s Advisory on E-cigarette Use Among Youth, 2018). The availability of flavored e-cigarettes is one of the most commonly cited reasons for e-cigarette use among youth (Tsai et al., 2018). Given the importance of flavors contributing to use of e-cigarettes among youth, these data briefs build on this previously published article, providing an update on trends in unit sales of e-cigarettes in the U.S. by product and flavor type. These retail sales data briefs are intended to provide information about population trends in sales for participating retailers; they do not include online sales or vape store sales and cannot be used to make conclusions about subgroup purchasing or behaviors. These estimates are based on the information available at the time of publication and may be subject to updates as more information becomes available.
Patients have reported high variability in substances/products they used in vaping, including both tobacco and tetrahydrocannabinol (THC) containing products as well as other products. Many e-cigarettes and vaping materials contain harmful or potentially harmful substances, and it is difficult to know what each individual product contains. The mislabelling of nicotine content in e-liquids has been previously addressed [8, 34]. Of note, several studies have detected nicotine in those e-liquids labelled as nicotine-free [5, 35, 36]. Among the 17 samples tested in this latter study 14 were identified to be counterfeit or suspected counterfeit. A third study detected nicotine in 7 of 10 nicotine-free refills, although the concentrations were lower than those identified in the previous analyses (0.1–15 µg/mL) [5].
Vaping nicotine has not been thoroughly evaluated in scientific studies. For now, not enough data exists on the safety of e-cigarettes, how the health effects compare to traditional cigarettes, and if they are helpful for people trying to quit smoking. The FDA monitors the national usage rates for all tobacco products, including an annual youth survey, and has seen a drastic increase in youth use of e-cigarette products in recent years. Due to what has been called an ‘epidemic’ of youth use of these products, FDA has prioritized prevention efforts.
Before and after the passage of SB 793, our researchers – all posing online as minors under the age of 21 – attempted to buy flavored e-cigarette products from 26 websites that sold them in California. Despite the known risks, vaping has seen a dramatic increase among U.S. adolescents. Researchers from Florida Atlantic University’s Schmidt College of Medicine conducted a study using data from the Youth Risk Behavior Survey which included participants from ninth to 12th grade. The lockdowns made the collection of CO readings difficult, particularly during 2 periods in 2022 shown in eFigure 2 in Supplement 2 when most study dropouts were recorded. This reduced the validated quit rates, but all 3 study arms were affected equally. This cluster of events is the likely reason for quit rates being lower than expected in our power calculations.
For use frequency pre-ban, respondents reported whether they used e-cigarettes daily (every day or most days in a week ) or weekly (at least once a week, but not most days). For post-ban, respondents reported whether they used e-cigarettes daily, weekly, less than weekly, or not at all. For both pre- and post-ban, respondents reported all e-cigarette flavors they used, the flavor that they primarily used, and the different ways they obtained e-cigarettes. Most respondents continued to use e-cigarettes with banned flavors post-ban. Compliance of local retailers with the ban was not high, and many respondents obtained banned-flavor e-cigarettes through legal channels. However, the significant increase in the use of non-flavored e-cigarettes post-ban suggests that these may serve as a viable alternative among those who used previously used banned or tobacco flavors.
However, they are a better alternative for adults than continued smoking. Vapes come in colorful, fun, and unique product designs, along with many flavors, all of which come together as part of the attraction for youth who believe that these products are not addictive and are safe to use. Many users believe vaping will reduce stress, anxiety, and depression.
When youth and young adults expose their brains to nicotine, they are vulnerable to nicotine addiction, may have trouble paying attention and concentrating, and experience mood disorders and reduced impulse control. Nicotine may alter the way their brains function for the rest of their lives. E-cigarette marketing, including product design and packaging, appeals to a young audience. For example, many e-cigarettes feature bright colors and fruit, candy, alcohol or other flavors that youth find attractive and interesting.
They contain nicotine, propylene glycol, glycerin, flavorings, and potentially dangerous chemicals like formaldehyde and various metals. Products containing nicotine or tobacco can’t be legally sold to people under age 21, but the government has faced challenges in regulating sales of disposable, imported devices. Some 10% of high school students said last year they had used e-cigarettes in the previous month, down from 14% the previous year, according to a national survey. About 5% of middle school students said they recently used e-cigarettes. During the ban, the city will study stores that sell vaping products, seek public input, look at what other communities have done and consider zoning changes, said Doug Plowman, planning director.
But levels are usually low and generally far lower than in tobacco cigarettes. Some of the harmful substances emitted in secondhand e-cigarette aerosols are similar to those in secondhand tobacco smoke. However, more research is needed to fully understand emissions from e-cigarettes and their impact on human health.

Scientists are still learning more about how e-cigarettes affect health. However, there is already enough evidence to justify efforts to prevent e-cigarette use by young people. JUUL is a popular brand of e-cigarette that is shaped like a USB flash drive. Like other e-cigarettes, JUUL is a battery-powered device that heats a nicotine liquid to produce an aerosol that is inhaled. According to the manufacturer, a single JUUL pod contains as much nicotine as a pack of 20 regular cigarettes. Although JUUL is currently the top-selling e-cigarette in the U.S., other e-cigarettes are becoming available that look like USB flash drives.
EVALI is a serious medical condition in which a person’s lungs become damaged from substances contained in e-cigarettes and vaping products. E-cigarettes invite harmful and unknown chemicals into the body and bloodstream. These cancer-causing agents reach deep into the lungs, irritating the bronchi and cause an increase in blood pressure, heart rate, as well as compromise adequate flow of blood to the heart and restrict of the arteries. Juul is probably the most popular brand of e-cigarette currently marketed. The device heats a nicotine-containing liquid pod to produce an aerosol inhaled by the user and any bystander within its radius (in the form of secondhand “smoke”). All Juul e-cigarettes have a high level of nicotine and in fact, according to the manufacturer, a single Juul pod contains as much nicotine as a pack of 20 regular cigarettes.

Effective July 1, 2019, people must be at least 21 years old to buy tobacco products in Illinois, including e-cigarettes. While they may help some people stop smoking, vaping products do not have FDA approval as a tool for quitting. E-cigarettes may even keep people from trying proven methods of quitting smoking. People who switch from traditional cigarettes to e-products may put off getting medical help or trying proven tools that can help with quitting. A comparison of the serum levels of nicotine from e-cigarette or conventional cigarette consumption has been recently reported [39].
Talk to your kids about e-cigarettes while they’re still willing to listen. In a recently published randomised trial of 886 subjects who were willing to quit smoking [100], the abstinence rate was found to be twice as high in the e-cigarette group than in the nicotine-replacement group (18.0% vs. 9.9%) after 1 year. Of note, the abstinence rate found in the nicotine-replacement group was lower than what is usually expected with this therapy. Nevertheless, the incidence of throat and mouth irritation was higher in the e-cigarette group than in the nicotine-replacement group (65.3% vs. 51.2%, respectively). Also, the participant adherence to the treatment after 1-year abstinence was significantly higher in the e-cigarette group (80%) than in nicotine-replacement products group (9%) [100]. Of note, another study indicated that although RANTES/CCL5 and CCR1 mRNA were upregulated in flavour/nicotine-containing e-cigarette users, vaping flavour and nicotine-less e-cigarettes did not significantly dysregulate cytokine and inflammasome activation [43].
Symptoms include vomiting, rapid heart rate, unsteadiness and increased salivation. The comprehensive review of the evidence finds that almost all of the 2.6 million adults using e-cigarettes in Great Britain are current or ex-smokers, most of whom are using the devices to help them quit smoking or to prevent them going back to cigarettes. It also provides reassurance that very few adults and young people who have never smoked are becoming regular e-cigarette users (less than 1% in each group). Youth use of e-cigarettes has surpassed youth use of regular combustible cigarettes in Delaware. E-cigarettes, which arrived in the United States around 2007, were originally intended for adults as a way to quit smoking traditional or “combustible” cigarettes.
They each have a mouthpiece, a battery, a heating element and a container for the vaping fluid, which is sometimes part of the mouthpiece. In the United States, marketing e-cigs to teens may have helped hook more young people on nicotine and tobacco. In August 2019, the FDA notified four companies to remove 44 flavored e-cigarette fluids and hookah tobaccos from sale in the United States. The agency crackdown underlined the FDA’s muscle-flexing on its authority to require FDA approval for new flavored products.
E-cigarette vapour has been reported to contain up to 7×1011 free radicals per puff.10 Both nicotine-free and nicotine-containing condensate induced a significant increase in ROS release from our AMs, which may explain the induction of apoptosis in nicotine-free liquid. There was significantly greater ROS production in AMs treated with nicotine-containing condensate than in nicotine-free condensate, once again suggesting both nicotine dependent and independent mechanisms at work. The effects of ECVC with and without were ameliorated by NAC and PI3K inhibition. A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. FDA’s award-winning public education campaign, “The Real Cost,” continues to prevent youth from tobacco initiation and use. Vaping involves using a device known as an e-cigarette—also called a vape pen, mod, or tank—to heat up a small amount of liquid, turning it into a vapor that can be inhaled. Most vape liquids contain substances such as propylene glycol and glycerol as base ingredients that create the vapor. But they may also contain other compounds including artificial flavors, nicotine, THC, and cannabinoid (CBD) oil.

Nicotine is harmful to the developing brain and the use of e-cigarettes as a teen increases the likelihood of smoking cigarettes as an adult. The chemical additives and flavoring can also cause unwanted health effects. The electronic cigarette (e-cigarette), for many considered as a safe alternative to conventional cigarettes, has revolutionised the tobacco industry in the last decades.
But more importantly, I do not want any other child to have to deal with what my son, Jacob dealt with and is still dealing with. His story is an important one because…People need to understand how quickly the addiction can happen. People need to understand how severe the negative effects of vaping are, both mentally and physically. I do not want any parent to have to hear his/her child have a panic attack on the phone like I did as Jacob sat locked in a bathroom stall at school because he couldn’t leave for fear he might vape with the other kids. E-cigarettes are nicotine-based products, and no nicotine use is safe. Until we know more, it is probably best to avoid these products whenever possible, including secondhand smoke. However, vaping early on may increase the chances of smoking ordinary cigarettes later in life.
The sale of tobacco products to minors is prohibited by law.This is an age restricted product and age verification is required at sale. Disturbances in AM function could therefore increase the risk of infection and enhance susceptibility to chronic obstructive pulmonary disease (COPD). We also assessed the effects of our ECVC with and without nicotine on human AMs to determine if this is a key component and determine whether anti-oxidants abrogate any of the effects. Objective Vaping may increase the cytotoxic effects of e-cigarette liquid (ECL). We compared the effect of unvaped ECL to e-cigarette vapour condensate (ECVC) on alveolar macrophage (AM) function.
“We’re at risk of dealing with nicotine addiction, several health concerns and potential lung diseases that we hadn’t really seen previously or at this rate from a particular product,” Dr. Joshua Mansour, a Los Angeles-based oncologist, told Drugwatch. Teen use of e-cigarettes has increased rapidly since the introduction of Juul in 2015. Surgeon General’s office found about 20 percent of high school students used an e-cigarette. The Surgeon General declared an “epidemic of youth e-cigarette use.” He pointed to surges in the number of teens using e-cigs and the dangers of nicotine to brain development in young people.
Pan and class one isoform phosphoinositide 3 kinase inhibitors partially inhibited the effects of ECVC/nfECVC on macrophage viability and apoptosis. Secretion of interleukin 6, tumour necrosis factor α, CXCL-8, monocyte chemoattractant protein 1 and matrix metalloproteinase 9 was significantly increased following ECVC challenge. Treatment with the anti-oxidant N-acetyl-cysteine (NAC) ameliorated the cytotoxic effects of ECVC/nfECVC to levels not significantly different from baseline and restored phagocytic function. E-cigarettes do not produce tar or carbon monoxide, two of the most harmful elements in tobacco smoke. There are different types of e-cigarette, including vape pens, vape bars, pod devices, mods lost vape review, and cigalikes.
Any smoking is harmful and you will only get the full benefits of vaping if you stop smoking completely. Some people manage to make a full switch very quickly, while for others it can take a bit longer. Nicotine itself is not very harmful and has been used safely for many years in medicines to help people stop smoking.
